6.2-If you are a consumer user, please note that we only provide our site for domestic and private use. You agree not to use our site for any commercial or business purposes, and we have no liability to you for any loss of profit, loss of business, business interruption, or loss of business opportunity.

All of our services are optional and you will be provided with either a “Medical Follow-Up” or “No-Further-Action” recommendation result on the day. You must be at least 18 years old at the time of booking your test ( if you are under 18 you must be accompanied by an appropriate adult) and you may be asked to provide identification before your appointment.

We do not do pregnancy scans. The practitioner performing your test will act upon their “duty of care” towards you and, with your permission, we will provide a copy report to your GP or other healthcare professional.

Cancelations and Rescheduling

9.1- You must email us, if you wish to cancel or reschedule your appointment giving a minimum notice of 24 hours at admin@theldc.uk

9.4- If you reschedule your appointment with more than 24 hours’ notice, you will not be charged any fees.

9.5- If you cancel your appointment with more than 24 hours’ notice, you will be NOT be charged an administration fee.

9.6- If your appointment is booked within 24 hours (same-day or next day), then any payments made is non-refundable and thus any cancellations of appointment will result in loss of the payments made.

9.7- If you are more than 15 minutes late for your appointment, your appointment is automatically cancelled and any payment you have made toward that appointment is Non-Refundable.

9.8- If you miss your appointment, you will be liable for full payment of your test and you will not be refunded for any fees / monies paid.

Late Attendance

10.1-Please arrive on time or earlier, as if you are late for your appointment (more than 15 minutes), unfortunately it will still end as normal at the allocated time and you will not be refunded.

10.2-If you miss your appointment, (See cancellations) you will be liable for full payment of your test.

Guests

A maximum of one guest is allowed at your appointment. Please refrain from bringing children under 12 as we have no facilities to care for them.

Price and payment terms

12.1-The price for the test will be the price published on our website price list published and updated from time to time on www.theldc.uk.

12.2-Payment shall be made before the test is done and we are not bound to deliver a test report until paid for by the customer.

Test report

13.1- We will supply a report via email in pdf format and in rare cases that you do not have an email you can request your report in paper format, including images and/or measurement results taken during the test. If a blood test result is also required to complete the test report, we or our authorised pathology laboratory partner will supply this to you within 7 working days of your appointment.

13.2-If our Practitioner recommends a medical follow-up as an outcome of the test, LDC recommends you to provide us in (writing or email), your GP’s email address.

With your permission, we will also send a copy of the test report directly to your -GP. If you don’t have one our own GPs will review and release the report

13.3-If, in our Practitioner’s opinion, a re-test appointment is needed to obtain the results for the primary purpose of the test, you will be advised on the optimum time for you to return. In most cases, one re-test appointment is offered free of charge, however this cannot be guaranteed. If you choose not to have a free re-test appointment, a minimum charge for the original test time will still apply.

Service provided

If London Diagnostic Centre provides a health test. Our Practitioners are not in possession of your medical records or history and are therefore not in a position to medically advise you on any diagnostic results. You should always seek the advice of your General Practitioner and / or Consultant with any questions you may have regarding the test, in particular if your symptoms persist.

Miscellaneous

15.1-Bookings for the tests are allotted a specific time slot. Whilst every effort is made to ensure the test is carried out at the allocated time, appointment times cannot be guaranteed.

15.2- London Diagnostic Centre accept no responsibility for any loss or damage to personal effects howsoever caused whilst customers are on the premises.

15.3- London Diagnostic Centre reserves the right to modify the service without notice from time to time. If, by doing so, the service is materially different then you will have a right of cancellation.

15.4- London Diagnostic Centre will not be responsible or liable for any incidental or consequential loss or damage:

• Caused by any act or omission by London Diagnostic Centre or its employees, servants or agents.
• Suffered by any person acting or failing to act as a result of the contents of any test or any information supplied in connection with the test. Nothing in this clause shall exclude or restrict any liability for death or personal injury arising from the negligence of London Diagnostic Centre.

15.5- London Diagnostic Centre will not be responsible for any breach of this contract caused by circumstances beyond its reasonable control.

The information you submit is taken as the truth and will form your medical information which we will keep in your file.

Clicking on the submit button on any of the forms below is same as signing that form and constitute as an “electronic signature”.

Disclaimer of Liability:

London Diagnostic Centre Limited ( hereby LDC) works very hard to offer an extremely high standard of service but we cannot guarantee that our users will always find everything to their liking. Children should always get permission from their parents before sending any information about themselves (such as their names, e-mail addresses, and phone numbers) , to us or to anyone else. We won’t knowingly allow anyone under 18 to use our services without consent from a legal guardian/parent.

UNDER NO CIRCUMSTANCES SHALL LDC, BE LIABLE TO YOU OR ANYONE ELSE FOR ANY DAMAGES ARISING OUT OF USE OF LDC, INCLUDING, WITHOUT LIMITATION, LIABILITY FOR CONSEQUENTIAL, SPECIAL, INCIDENTAL, INDIRECT, OR SIMILAR DAMAGES, EVEN IF WE ARE ADVISED BEFOREHAND OF THE POSSIBILITY OF SUCH DAMAGES. YOU AGREE THAT THE LIABILITY OF LDC IF ANY, ARISING OUT OF ANY KIND OF LEGAL CLAIM IN ANY WAY CONNECTED TO THE SERVICE SHALL NOT EXCEED THE AMOUNT YOU PAID TO THE SERVICE FOR THE USE OF THE SERVICE.

WHILST LDC PROVIDES AN EXTREMELY CARING AND THOROUGH DIAGNOSTIC SERVICE WE CANNOT GUARANTEE THAT OUR SERVICE WILL CURE OR PREVENT DISEASE OR GUARANTEE FUTURE HEALTH.

LDC DOES NOT RECOMMEND SELF-DIAGNOSIS OR SELF-MEDICATION, AND NO INFORMATION WITHIN THIS WEBSITE OR PRESENTED BY LDC MAY BE CONSTRUED OR INTERPRETED AS RECOMMENDING SELF-DIAGNOSIS OR SELF-MEDICATION. INDIVIDUALS WITH HEALTH DISORDERS, MEDICAL CONDITIONS, OR ANY CONDITION NEEDING MEDICAL SUPERVISION ASSUME FULL RESPONSIBILITY FOR OBTAINING SUCH PROFESSIONAL MEDICAL ASSISTANCE.

LDC HAS A ZERO TOLERANCE POLICY TO AGGRESSIVE AND VIOLENT BEHAVIOUR TO ANY STAFF ASSOCIATED WITH ITS SERVICES.

BY USING LDC, YOU AGREE TO ABIDE BY THE ABOVE TERMS AND CONDITIONS AND HAVE READ AND UNDERSTOOD THE DISCLAIMERS OF LIABILITY.
